The Valsalva manoeuvre is an internationally recommended treatment for supraventricular tachycardia, but cardioversion is rare in practice (5–20%), necessitating the use of other treatments including adenosine or even electrical therapies. The authors of this article published in 2015, emergency doctors and cardiologists from England, propose a modified manoeuvre and evaluate its effectiveness in a randomized study with 433 participants enrolled. They find that the conversion rate to a sinus rhythm is higher (43% vs. 17%) than using the standard Valsalva manoeuvre and propose, consequently, to use the modified Valsalva instead of the classical one. You can see, in the video attached, how to do a modified Valsalva manouevre, and download the article in the following link: https://drive.google.com/file/d/1sUHfi1VQXaUX1J298hN8pYdM1USdeIxZ/view?usp=sharing

High-flow nasal cannula therapy (HFNC) is recognized as the first therapeutic option in patients with hypoxemic acute respiratory failure (ARF), and there is evidence of its efficacy also in patients with hypercapnic ARF. There is a group of elderly (more than 75 years) patients with ARF who, after receiving medical treatment and conventional oxygen therapy, remain with dyspnoea and refractory hipoxemia. Many of them, due to factors such as age, severe comorbidity, functional capacity or willingness, are not candidates to be admitted to the intensive care unit. This study, developed by a group of emergency doctors from Spain, newfangled because the subjects were all elderly patients as defined above, included 200 patients along a year (mean age, 83 years), and concludes that HFNC is an effective and safe option in these patients as a therapeutic ceiling, with benefits regarding to oxygenation, respiratory rate and dyspnoea but also to comfort.
While fluid administration
is one of the most common therapeutic interventions in critical care, it’s
not harmless at all. Fluids should be regarded as drugs with specific
indications, contraindications, and potential adverse effects. The renewed
concept of “fluid stewardship”, analogous to antibiotic stewardship, focusses
on the 4 D’s (drug, dose, duration, and de-escalation), the 4 questions (when
to start and when to stop fluid therapy, and when to start and when to stop
fluid removal), the 4 indications (resuscitation, maintenance, replacement and
nutrition), and the conceptual ROSE model
describing 4 fluid phases (resuscitation, optimization, stabilization and
evacuation).
The term deresuscitation was coined in 2014 and defined as active fluid removal in patients with fluid overload using drugs and/or ultrafiltration (UF). The term ‘de-escalation’ relates more to reducing the amount and rate of previously started fluid administration. The article web ring today, published in 2022 for five intensive care doctors from different countries, pretends (succesfully, we think) to explain all we should know about the adverse effects of the fluid overload and how and when to start (and finish) the deresuscitation.
Practice guidelines provide clear evidence-based
recommendations for the use of drug therapy to manage pain, agitation, and
delirium associated with critical illness. Dosing recommendations however are
often based on strategies used in patients with normal body habitus.
Recommendations specific to critically ill patients with extreme obesity are
lacking, because recommendations for medication dosing in critically ill obese patients
are not available from adequately powered randomized studies with clinically
relevant endpoints. The authors of this article, researchers from the
Midwestern University’s College of Pharmacy (Arizona, USA), conducted a detailed literature search,
and propose strategies for dosing analgesics, sedatives and anti-psychotics in patients
with obesity and extreme obesity. Download link: https://drive.google.com/file/d/1UtW-odhQS6F0Bnbci3jTRep3JHuX3jN3/view?usp=sharing27 diciembre 2022

https://drive.google.com/file/d/158mXv4R3WZzO4YJdEbHGDyYm6VYz3aPz/view?usp=sharing
Targeted temperature management (TTM) is a technique which
has been in use as a part of the post-cardiac arrest care for almost 20 years,
since the first major reports on its benefits in improving out-of-hospital and
in-hospital cardiac arrest survival as well as improving neurological outcome.
However, the findings from one of the biggest trials to date in this topic, the
TTM2 trial, surprisingly showed no difference in mortality between patients
after out-of-hospital cardiac arrest who underwent normo- or hypo-thermia. This
results, far from contraindicate its use, forces us to re-evaluate with new
perspectives this technique in order to discover why the demonstrated beneficial
effects in animal models aren’t replicated in humans. Download link:
